With your help, NAVRMA hopes to have an excellent and safe 2021 Conference. During this COVID-19 pandemic your health and safety is of the utmost importance to us and Colorado State University where the event is being held. In these ever-changing times, we will adhere to strict guidelines of the University, Colorado Department of Public Health, and the CDC. Attached please find the CSU C. Wayne McIlWraith TMI Event Guidelines and Safety Plan (information copied below), Liability Release Form, Photo Release Form, and the COVID-19 Visitor-Vendor-Guest Screening Checklist for your reference.
Please read through all the information included on this page and complete and email the following to Sam Mallouf (email@example.com), CSU TMI event coordinator, PRIOR to the start of the meeting to facilitate registration:
i. Acknowledgement Form
ii. Liability Release Form
iii. Photo Release Form
iv. Documentation verifying you are at least 2 weeks post final SARS-CoV-2 vaccination dose OR proof of a negative SARS-CoV-2 test (antigen or PCR) collected within 72 hours of travel to CSU.

The Translational Medicine Institute (TMI) requires that all Visitors complete the COVID Acknowledgement Form PRIOR to arriving to an in-person event on campus, if while in attendance, any person exhibits signs of illness, weakness or susceptible behaviors, they will be required to leave the premises.
Safety is our first, and by far our major concern. Protecting the physical, as well as the emotional health of TMI visitors, TMI staff and faculty, and the CSU/Fort Collins Community is the top priority. We will follow Local, State, and National Public Health Authority directives.
